New Patients – Tulsa, OK

A Lifetime of Smiles Starts Here

Our goal at My Dentist Midtown isn’t to just give you one or two good appointments, but rather provide a lifetime of exceptional care and service you’ll come to rely on. Before your first visit, you’ll find everything you need to know ahead of time below, and if you have any questions or concerns you’d like to share with us, don’t hesitate to give us a call!

Your First Dental Visit

Rather than revolving around you filling out forms and checking boxes, your first visit will primarily focus on one thing—communication. We want to get to know you as a person and fully understand what you hope to accomplish by coming to see us, so a big part of your time will be spent having a conversation with Dr. Flinchum about your expectations. On top of this, she’ll also perform an exam and go over her findings with you so you both know where you’re starting when it comes to your oral health.

New Dental Patient Forms

You don’t need to wait until you’re sitting in our reception area to fill out our patient forms—you can take care of them right now in the comfort of home! Just download them below, add in the information, and bring them to your appointment so we can check you in and bring you to the back nice and fast.
